This page covers most everything you might need to know before your first session. Please read through this page and fill out all forms linked below in advance of your session.
New client forms - must be completed before treatment can begin
Please fill out the following well in advance of your first appointment, ideally at least 24 hours in advance. If you have any questions about something on a specific form, just bring your question with you and we can address it during your session.
These forms can take 10-20 minutes, so please leave time to complete them before your appointment starts. Linked below are electronic forms that you can fill out online. If you prefer paper, you can print and complete this packet instead (six pages).

Office policies, including detailed cancellation and financial policies, may be accessed here.
As a reminder, I require two business days' notice to change or cancel appointments (more details below). Please keep your confirmation email for links to reschedule or cancel your appointment. If you need to change your appointment, you may do so up to two business days’ before your appointment.
Location
We will meet in person, unless otherwise specified. In cases of illness or inclement weather, we will meet online.
Clinic for In-Person Appointments:
The office is located at 200 NE 20th Ave, Suite 120 (between Davis and Everett). You may park in the parking lot in front of the building. The reception area is not a waiting area, so if you arrive early please wait outside (or in your vehicle).
Please knock at the time of your appointment and your provider will meet you at the door. To protect each other and our immune-compromised patients, please wash your hands or use hand sanitizer once inside.
I ask folks to leave shoes in the waiting area in order to cut down on allergens and pollutants; I have clean slippers you may use or you are welcome to bring house shoes or socks.
As I have autoimmune conditions and work with others who are immune-compromised, please plan to mask if you are symptomatic (or have been exposed to someone who is).

See your confirmation and reminder emails for the Google Meet link for online appointments. If you are joining from a mobile device, please download the app ahead of time.
Please be sure you are in a comfortable and private place for our session where you will not be disturbed.
